Applying for Services

Every eligible individual can get one $500 respite voucher per year. In order to be considered for In-Home Respite – fill out and submit a signed In-Home Respite Application as well as a Release of Liability. These also need to be filled out yearly if/when you want to renew. These can be filled out and mailed to the following address:

Special Circumstances

There is a mechanism to get a 2nd and 3rd $500 voucher within the same year – called the 1st Special Circumstance and 2nd Special Circumstance. These have a different application from the regular Respite Application.
